By completing this form, you are attesting that you have completed the NAHAM Proctor webinar with a passing score of 80% or more and adhere to all of the requirements stated therein. You are also agreeing to the following:


I have received and read the NAHAM Proctor Manual and Examination Rules, and agree to the following:


  • To allow no one access to any NAHAM examination, or to duplicate or retain any part of any examination without prior written approval from NAHAM
  • To administer the examination following NAHAM examination administration policies and protocol, as stated in the Proctor Manual
  • To assume full responsibility for maintaining the total security of individual examination materials while they are in my possession, from time test materials are received from the board or NAHAM, through examination administration and return to Schroeder Measurement Technologies.
  • To not disclose or provide to anyone - directly or indirectly - any information or secure materials to which I have been made privy during the administration of the NAHAM Certified Healthcare Access Associate (CHAA) or Certified Healthcare Access Manager (CHAM) examination.
  • To keep all documents, data, exam related materials as confidential information received from NAHAM and shall remain the exclusive property of NAHAM and that all documents, data, exam related materials and information shall be returned promptly to NAHAM upon completion of service.
  • I further affirm that my service to NAHAM creates no known or perceived conflicts of interest, and that should any arise, I will immediately recuse myself from serving as an exam proctor.
  • I affirm that I am currently certified as a CHAA or CHAM certificant for the exam that I am proctoring OR
  • I attest that I will abstain from testing for the exam in which I am proctoring for a minimum of three years from the exam date.

Failure to comply with the guidelines as provided in the proctor webinar and proctor handbook will result in losing all privileges to proctor future NAHAM examinations and potential disciplinary action by the Certification Commission.
