CHAM Retroactive Certification Maintenance Application

Thank you for participating in the CHAM Retroactive Certification Maintenance program.


The first step is to fill out the following form with the hours that you earned during your missed renewal cycle. For each missed cycle, you need to submit 60 professional development hours. 

Professional development activities (continuing education) must be documented and submitted for the educational time frame(s) that would have been filed had you participated in the regular Certification Maintenance Program. 


Each professional development hour needs to include the following information:

  • Date of activity
  • Title/description of activity
  • Number of hours earned for activity

If you are unsure about which cycle(s) you have missed please reference your Certification Central dashboard for your expiration date or reference the NAHAM website for more information.


Once your hours have been approved you will receive an email from the NAHAM staff with information on how to pay the $175 (members) or $350 (nonmembers) fee.
