Apply for Contact Hour Approval

NAHAM will accept and review applications for continuing education units for Affiliate and other similar educational meetings.

Applications and accompanying material must be received at least 30 days prior of the meeting presentation to receive NAHAM approval. 

There is no fee to NAHAM affiliates or Business Partner members if the application is received in NAHAM offices at least 30 days prior to the program.  If the completed application is not received at least 30 days prior to the start of the program NAHAM will assess a $100 late fee.  If the late fee is not paid the organization's future contact hour applications will not be reviewed.

There is a $200 annual application fee per program for all other organizations.  Please include a check payable to NAHAM in this amount along with the completed application form. If you have any questions about completing the application, please refer to the sample application listed on the left side of this page. 

All applications must be completed in full and on the NAHAM Contact Hour Application form.  Applications received that are not properly completed will not be reviewed.

Please note that approvals for NAHAM Contact Hours are valid only for the approved educational meeting.  Credit cannot be granted retroactively and all applications must utilize the official application form located on this website.
